Research progress on application of advance care planning in elderly patients with chronic heart failure

Abstract：Elderly patients with heart failure are prone to relapse,have a heavy burden of symptoms,a high risk of sudden death,and often faced with medical decisions that were inconsistent with their personal wishes,so it is particularly important to implement advance care planning for them.This paper reviewed the concepts related to advance care planning,the necessity of applying advance care planning to elderly patients with chronic heart failure,the forms and effects of advance care planning in elderly patients with chronic heart failure,the challenges and suggestions of implementing advance care planning for elderly patients with chronic heart failure,in order to provide reference for the development of advance care planning and hospice care of elderly patients with chronic heart failure in China.
