Applied study on hierarchical and continuous care based on Triangle model in patients with type 2 diabetes

Abstract:Objective:To analyze the effect of triangle-based hierarchical continuation care in patients with type 2 diabetes mellitus(T2DM).Methods:A total of 120 T2DM patients treated in Affiliated Suqian Hospital of Xuzhou Medical University from November 2021 to November 2022 were included and divided into observation group and control group according to random number table method,with 60 cases in each group.The patients in control group received continuous care,and the patients in observation group received hierarchical continuous care based on Triangle model.Blood glucose control,self-management ability and quality of life were assessed before intervention,at 3 months and 6 months after intervention.The number of readmissions during the intervention period was also recorded.Results:After 3 months and 6 months of intervention,fasting blood glucose(FPG),postprandial 2 hours plasma glucose(2 h PG),and glycated hemoglobin(HbA1c)levels in both groups were significantly lower than those before intervention,the score of the Summary of Diabetes Self-Care Activities Questionnaire(SDSCA)was higher than that before intervention,and the score of Chinese Diabetes Specific Quality of Life Scale(DSQL)was lower than that before intervention.The improvement of all indexes in observation group was better than that in control group(P<0.05).However,there was no significant difference between the two groups at 6 months intervention and 3 months intervention(P>0.05).The number of re-hospitalization in observation group was less than that in control group(P<0.05).Conclusion:Triangle-based hierarchical continuous care can improve glycemic control,self-management ability and quality of life in T2DM patients,and reduce the number of re-hospitalizations.
