Transitional care services for neoplastic hematologic disorder patients in ethnic inhabited areas of China

Abstract:Objective:To study the methods and effects of transitional care on patients with the neoplastic hema-tologic disorder in ethnic minority areas.Methods:A total of 100 patients were divided into experiment group (50 cases)and control group (50 cases)according to the patients demands.All of them received conventional guides when they discharged.Nurses capable of both Uighur language and Chinese did transitional care on ex-periment group through phone calls,QQ platform,WeChat platform,and health education classes.After a month,the mastery of diseases related knowledge and satisfaction degrees on nursing work were evaluated and compared.Results:The scores of disease related knowledge mastery and nursing satisfaction in the experiment group were significantly higher than the control group (P <0.05)Conclusion:The transitional care on patients with the neoplastic hematologic disorder in ethnic inhabited areas of China can improve both the quality of life and the prognosis.
