Influence of collaborative care model on health behaviors and self care abil ity of community stroke risk groups
Abstract:Objective:To probe into influence of collaborative care model on health behaviors and self care ability of community stroke risk groups. Methods:A total of 80 of community stroke risk individuals were randomly divided into control group and intervention group,40 cases in each.The ca-ses in intervention group received collaborative care and the cases in control group received community routine care.Health Promoting Lifestyle Pro-file and Exercise of Self Care Agency Scale were used to evaluate health behaviors and self care ability of community stroke risk individuals re-spectively before and after intervention.Results:After 6 months of inter-vention,there were statistically significant differences in the scores of health behaviors and self care ability between both groups (P < 0.05 ). Conclusion:Collaborative care model can significantly improve health be-haviors and self care ability of community stroke risk groups.
