Evaluation on effect of personal ized outpatient visits intervention for discharged patients with diabetes

Abstract:Objective:To probe into effect of personalized outpatient visits intervention for glucose metabolism and self management level of dis-charged patients with type 2 diabetes.Methods:A total of 160 cases of dis-charged patients with type 2 diabetes were randomly divided into observa-tion group and control group,80 cases in each.Patients in control group received conventional diabetes follow up,patients in observation group re-ceived six months of personalized clinic follow up and intervention,then to compare the changes of body mass index (BMI),glycated hemoglobin (HbA1c),glucose,total cholesterol,triglycerides and self care behaviors in two groups of patients before intervention and 6 months after interven-tion.Results:After intervention,HbA1c,blood glucose and self care be-havior scores in both groups had significant change than before interven-tion,and the difference was statistically significant (P<0.05);after the in-tervention,there was statistically significant difference in HbA1c,2 h post-prandial blood glucose and self care behavior scores between both groups (P<0.05 ),that in observation group was better than that in control group.Conclusion:personalized visit interventions can significantly im-prove their self management level,and can improve the level of glucose metabolism in patients to some extent.
