Early Assessment of Left Heart Function Changes in Patients with Ischemic Stroke

Abstract:Objective To explore the early evaluation methods for left ventricular function changes in patients with ischemic stroke. Methods A total of 65 patients with ischemic stroke who were admitted to Binzhou People's Hospital, Shandong First Medical University from October 2022 to June 2023 were selected. After admission, all patients underwent tests including total cholesterol (TC), blood glucose, cardiac troponin (cTn) T, N-terminal pro-B-type natriuretic peptide (pro-BNP), electrocardiogram (ECG), and echocardiography, as well as left ventricular myocardial work analysis using pressure-strain loop technology within 12 hours. Based on clinical symptoms and auxiliary examination results, patients were divided into two groups according to whether they had left ventricular function changes: the left ventricular function abnormal group and the left ventricular function normal group. Univariate analysis and multivariate logistic regression were used to compare clinical characteristics and auxiliary examination results between the two groups. The receiver operating characteristic (ROC) curve was used to analyze the predictive parameters for left ventricular function changes in the early stage of ischemic stroke. Results According to whether there were changes in left ventricular function, the patients were divided into the left ventricular function abnormal group (n=36) and the left ventricular function normal group (n=29). There were significant differences in TC, QT interval, GWI, GCW, GWW, GWE, and heart rate between the two groups (P<0.05). Multivariate logistic regression analysis showed that GWI, GCW, GWW, and GWE within 12 hours after admission were independent risk factors for left ventricular function changes in ischemic stroke (P<0.05). The ROC curve showed that GWW had good predictive value for left ventricular function changes in ischemic stroke, with an AUC of 0.868. Conclusion Pressure-strain loop technology has high predictive value in evaluating left ventricular function changes in the early stage (within 12 hours) of ischemic stroke.
