Analysis of Risk Factors in Elderly Patients with Pneumonia Secondary to Antibiotic-associated Diarrhea

Abstract:Objective To explore the risk factors for antibiotic-associated diarrhea (AAD) in elderly patients with pneumonia. Methods A retrospective analysis was conducted on 112 elderly pneumonia patients admitted to the Qingcun Community Health Service Center, Fengxian District, Shanghai City, from March 2021 to February 2024. The occurrence of AAD in these patients was recorded, and univariate and multivariate logistic regression analyses were used to identify the risk factors for AAD. Results Among the 112 elderly pneumonia patients, 38 (33.93%) developed AAD. Univariate and multivariate logistic regression analyses revealed that age ≥80 years (OR=3.333), severe pneumonia (OR=3.370), fasting (OR=3.808), no prophylactic use of probiotics (OR=1.826), use of proton pump inhibitors (OR=2.942), combined use of antibiotics (OR=2.389), and prolonged antibiotic use for ≥7 days (OR=3.178) were independent risk factors for AAD in elderly pneumonia patients (P<0.05). Conclusion The risk factors for AAD in elderly patients with pneumonia include age, disease severity, fasting, lack of prophylactic use of probiotics, use of proton pump inhibitors, combined antibiotic use, and prolonged antibiotic treatment duration.
