The correlation between serum TGF-β1,GP-73,CCL11,TLR4 levels and platelet-albumin-bilirubin score in patients with HBV infection-related cirrhosis

Abstract:Objective To analyze the correlation between serum levels of transforming growth factor-β1(TGF-β1),Golgi protein-73(GP73),C-C Motif Chemokine Ligand 11(CCL11),Toll-Like Receptor 4(TLR4)and platelet-albumin-bilirubin(PALBI)score in patients with hepatitis B virus(HBV)infection-related cirrhosis.Methods A total of 265 patients with chronic HBV infection admitted to our hospital from January 2021 to December 2024 were prospectively selected and divided into cirrhosis group and non-cirrhosis group according to whether cirrhosis occurred.All patients underwent serological tests,and the PALBI score was calculated.Spearman was used to analyze the correlation between serum TGF-β1,GP-73,CCL11,TLR4 levels and PALBI score in patients with HBV infection-related cirrhosis.Univariate and multivariate logistic regression were used to analyze the influencing factors of cirrhosis in patients with chronic HBV infection.The area under the curve(AUC)of the receiver operating characteristic(ROC)curve was used to analyze the predictive value of the above serological indicators for cirrhosis in patients with chronic HBV infection.Results Among the 265 patients with chronic HBV infection,84(31.70%)were in the cirrhosis group and 181(68.30%)were in the non-cirrhosis group.Spearman correlation analysis showed that the levels of serum TGF-β1,GP-73,CCL11 and TLR4 in the cirrhosis group were positively correlated with PALBI score(r=0.633,0.669,0.581,0.659,P<0.05).Multivariate logistic regression analysis showed that drinking history,antiviral treatment or not,ALT,Child-Pugh classification,PALBI score,TGF-β1,GP-73,CCL11 and TLR4 were the influencing factors of cirrhosis in patients with HBV infection.ROC results showed that the AUC of the combined detection of the above factors was 0.913(95%CI:0.876~0.950).The AUC was significantly higher than that of each single test[0.587(95%CI:0.512~0.662),0.648(95%CI:0.576~0.721),0.754(95%CI:0.693~0.814),0.578(95%CI:0.506~0.651),0.732(95%CI:0.673~0.791),0.779(95%CI:0.715~0.843),0.734(95%CI:0.665~0.804),0.781(95%CI:0.721~0.842),0.745(95%CI:0.680~0.810)],and the differences were statistically significant(Z=8.532,7.280,4.351,8.080,4.836,3.504,4.998,5.672,P<0.001).Conclusion The serum levels of TGF-β1,GP-73,CCL11 and TLR4 in patients with chronic HBV infection-related cirrhosis were positively correlated with PALBI score.The combined detection of the above factors can effectively predict the risk of cirrhosis and the degree of disease progression in patients with chronic HBV infection.
