Evaluation of the effectiveness of rosuvastatin combined with intermittent exercise in preventing in-stent restenosis after carotid artery stenting for atherosclerosis

Abstract:Objective To investigate the effect of aerobic interval exercise combined with rosuvastatin and other medications in preventing in-stent restenosis(ISR)after carotid artery stenting(CAS).Methods From February 2022 to January 2023,120 patients who underwent carotid artery stenting for carotid artery stenosis were enrolled.They were randomly divided into a medication group and a combined exercise group,with 60 patients in each group.The medication group received daily oral rosuvastatin postoperatively,while the combined exercise group initiated aerobic interval training 4 weeks after surgery.Each exercise session lasted 25 minutes,including 5 minutes of warm-up,15 minutes of aerobic interval exercise,and 5 minutes of cool-down∕stretching,performed 3 times weekly for over 48 weeks.Carotid artery CTA was used to assess intravascular restenosis.The incidence of ISR,target vessel revascularization,mortality,and other major adverse events were compared between the two groups at 12 months.Levels of C-reactive protein(CRP),interleukin-6(IL-6),total cholesterol(TC),triglycerides(TG),low-density lipoprotein cholesterol(LDL-C),and adiponectin(APN)were measured before and after the intervention.Regularly monitor liver and kidney function,blood glucose,and other indicators to observe medication safety.Results In the medication group,one case was lost to follow-up.The in-stent restenosis rate in the combined exercise group was 1.67%,lower than that in the medication group(10.16%),with χ2=3.884 and P=0.048.There were no statistically significant differences in target vessel revascularization rate or mortality between the two groups.After intervention,the CRP and IL-6 levels in the combined exercise group were(6.8±2.2)mg∕L and(7.5±2.9)ng∕L,respectively,both lower than those in the medication group(7.9±2.8)mg∕L and(8.6±2.3)ng∕L.The APN level in the combined exercise group was higher than that in the medication group.For CRP and IL-6,the comparisons yielded(t=2.385,2.290,P<0.05),respectively.After intervention,the TC(3.9±0.4)mmol∕L,TG(1.3±0.4)mmol∕L,and LDL-C(2.4±0.4)mmol∕L levels in the combined exercise group were all lower than those in the medication group(4.6±0.6)mmol∕L,(1.5±0.3)mmol∕L,and(2.6±0.5)mmol∕L(t=7.499,3.081,2.411,P<0.05),respectively.No serious adverse reactions were observed during the monitoring period.Conclusion Compared to pharmacotherapy alone,the combination of intermittent exercise and pharmacotherapy exerts a more significant effect in reducing inflammatory responses,improves lipid profiles,and decreases the incidence of ISR following CAS.
