The correlation between CPR duration,platelet count and end-tidal carbon dioxide partial pressure monitoring and short-term prognosis after spontaneous circulation recovery in hospitalized patients with cardiac arrest

Abstract:Objective To analyze the correlation between cardiopulmonary resuscitation(CPR)duration,platelet count and end-tidal carbon dioxide partial pressure(PETCO2)monitoring and short-term prognosis after return of spontaneous circulation(ROSC)in hospitalized patients with cardiac arrest.Methods A total of 202 cardiac arrest patients with return of spontaneous circulation(ROSC)treated at Yuncheng Central Hospital Affiliated to Shanxi Medical University from January 2023 to December 2024 were enrolled.They were divided into groups based on clinical indicators:long resuscitation group(n=122,≥15 min)and short resuscitation group(n=80,<15 min);high platelet group(n=79,≥100×109∕L)and low platelet group(n=123,<100×109∕L);high PETCO2 group(n=77,≥27.5 mmHg)and low PETCO2 group(n=125,<27.5 mmHg).Kaplan-Meier curves were used to analyze the relationship between overall survival and different groups of CPR duration,platelet count,and PETCO2,with Log-Rank test for survival differences between groups.Univariate Cox regression was applied to screen short-term prognostic factors,followed by multivariate Cox regression for independent predictors.The predictive value of single and combined indicators for poor short-term prognosis in ROSC patients was evaluated using the area under the receiver operating characteristic(ROC)curve(AUC),with Delong test for statistical comparison.Results Kaplan-Meier analysis showed that the median survival time in the long resuscitation group,low platelet group,and low PETCO2 group was significantly lower than that in the short resuscitation group,high platelet group,and high PETCO2 group,with statistically significant differences(P<0.05).Multivariate Cox regression analysis indicated that the Acute Physiology and Chronic Health Evaluation Ⅱ score on admission,Glasgow Coma Scale score on admission,CPR duration,platelet count,and PETCO2 were independent influencing factors for poor short-term prognosis in cardiac arrest patients with ROSC(P<0.05).The ROC curve results showed that the areas under the curve(AUC)for combined detection of CPR duration,platelet count,and PETCO2,as well as single indicators,were 0.873(95%CI:0.822~0.924),0.712(95%CI:0.638~0.787),0.763(95%CI:0.680~0.845),0.787(95%CI:0.720~0.854),0.733(95%CI:0.662~0.804),and 0.645(95%CI:0.564~0.726),respectively.Delong test showed that the AUC of combined detection was significantly higher than that of each single indicator(Z=4.245,3.347,3.165,3.109,4.863;P<0.05).Conclusion CPR duration,platelet count,and PETCO2 monitoring can effectively predict the short-term prognosis of cardiac arrest patients with ROSC,providing an objective basis for early identification of high-risk patients.This finding helps optimize post-resuscitation management strategies and lays a data foundation for establishing a multi-index combined prognostic assessment model.
