The clinical predictive value of Atrogin-1 and BMI in sarcopenia,base on a retrospective cohort study analysis

Abstract:Objective This study aimed to investigate the roles of Atrogin-1 and body mass index(BMI)in the clinical prediction of sarcopenia,evaluate their independent influencing factors and combined predictive efficacy,and provide scientific evidence for early diagnosis and risk assessment of sarcopenia.Methods A retrospective cohort study was conducted,enrolling 114 eligible patients between August 2022 and October 2024,including 63 sarcopenia cases and 51 non-sarcopenia controls.Demographic and clinical characteristics were collected,and multivariate binomial logistic regression was performed to explore the effects of BMI and Atrogin-1 on sarcopenia.Trend analysis was used to validate the stability of Atrogin-1 and BMI across different models,and receiver operating characteristic(ROC)curve analysis was applied to assess their predictive performance.Results Baseline data revealed significant differences between sarcopenia and non-sarcopenia groups in age,weight,BMI,appendicular skeletal muscle mass index(ASM),calf circumference,Atrogin-1 levels,diabetes history,and frailty status.Multivariate logistic regression identified BMI and Atrogin-1(OR=0.68,1.08,P<0.001)as independent influencing factors for sarcopenia.Risk factor validation models demonstrated that Atrogin-1 exhibited an upward risk trend with increasing values,while BMI showed a downward risk trend with higher values,regardless of adjustments for age,sex,smoking,alcohol consumption,diabetes,hypertension,or metabolic indicators.ROC analysis revealed that the AUC for BMI in predicting sarcopenia was 0.83(0.75~0.90),while Atrogin-1 had an AUC of 0.84(0.74~0.94).The combined predictive model achieved a higher AUC of 0.91(0.85~0.98),outperforming individual indicators.Conclusion Both Atrogin-1 and BMI are independent predictors of sarcopenia and can effectively assess its risk.The combined predictive model demonstrates superior accuracy compared to individual indicators,suggesting that integrating Atrogin-1 and BMI may enhance early identification of sarcopenia,offering valuable insights for clinical screening and intervention.
