A comparative study on the efficacy of erector spinae plane block combined with general anesthesia versus conventional general anesthesia in patients undergoing posterior lumbar interbody fusion surgery

Abstract:Objective To investigate the effects of erector spinae plane block combined with general anesthesia on pain relief,delirium,and muscle relaxant usage in patients undergoing posterior lumbar fusion with internal fixation.Methods A retrospective study was conducted on 98 patients who underwent lumbar fusion with internal fixation at our hospital from January 2024 to July 2024.Patients were divided into a control group(n=53)receiving standard endotracheal intubation general anesthesia and a block group(n=45)receiving erector spinae plane block combined with general anesthesia.The use of muscle relaxants,pain levels,Quality of Recovery-15 score(QoR-15),post-anesthesia care unit(PACU)conditions,and adverse reactions were compared between the two groups.Results There were no significant differences in baseline characteristics between the two groups(P>0.05).Heart rate(HR)and mean arterial pressure(MAP)in the block group were significantly lower than those in the control group during T1~T4 periods(tHR=4.034,3.662,4.000,3.095;tMAP=3.382,3.681,2.320,2.983,P<0.05).The block group showed shorter operation time,anesthesia duration,less blood loss,fluid infusion volume,sufentanil dosage,remifentanil dosage,rocuronium dosage,and propofol dosage compared to the control group(t=2.068,2.058,3.396,2.165,2.487,13.422,2.725,5.313,P<0.05).Postoperative VAS scores at 2,12,24,and 36 hours were significantly lower in the block group(t=7.374,8.616,6.953,4.267,P<0.05).QoR-15 scores were significantly higher in the block group on postoperative days 1 to 3.Extubation time and PACU stay duration were significantly shorter in the block group(t=3.676,3.002,P<0.05).The incidence of adverse reactions was significantly lower in the block group(4.44%)compared to the control group(χ2=4.712,P<0.05).Conclusion The application of erector spinae plane block combined with general anesthesia in lumbar fusion with internal fixation significantly reduces HR and MAP,decreases operation and anesthesia times,lowers intraoperative blood loss and fluid infusion volumes,and reduces the usage of anesthetic drugs such as sufentanil,remifentanil,and rocuronium.Additionally,this anesthesia method effectively alleviates postoperative pain,improves recovery quality scores,shortens extubation and PACU stay times,and decreases the incidence of adverse reactions.
