The expression of PCT,MMP-9,Preepsin and NGAL in patients with multiple organ dysfunction syndrome secondary to emergency sepsis

Abstract:Objective To study the influencing factors of multiple organ dysfunction syndrome in emergency sepsis patients and the expression significance of plasma soluble CD14 subtype(Presepsin),neutrophil gelatinase-related lipocalin(NGAL),serum procalcitonin(PCT)and matrix metalloproteinase-9(MMP-9).Methods The clinical data of 197 emergency sepsis patients admitted to the Second Affiliated Hospital of Xi'an Jiaotong University from January 2020 to February 2024 were retrospectively included,and they were divided into the group without multiple organ dysfunction(n=134)and the group with multiple organ dysfunction(n=63);the prognosis of patients with multiple organ dysfunction group was followed up for 28 d and divided into the survival group(33 cases)and the death group(30 cases).To analyze the single factor and influencing factors of multiple organ dysfunction syndrome(MODS)in emergency sepsis patients,and to analyze the diagnostic value by using the receiver's operating characteristic(ROC)curve.The levels of plasma Presepsin,NGAL,serum PCT and MMP-9 were compared between the survival group and the death group.Results Compared with the non-multiple organ dysfunction group,the scores of sequential organ failure(SOFA),the levels of plasma Presepsin,NGAL,serum PCT,C-reactive protein(CRP),MMP-9 and creatine phosphokinase isoenzyme(CK-MB)were higher in the multiple organ dysfunction group(P<0.05).The high score of SOFA,levels of plasma Presepsin,NGAL,serum PCT and MMP-9 were all independent influencing factors for the secondary multiple organ dysfunction syndrome in emergency sepsis patients(P<0.05).The ROC curve analysis showed that the area under the curve(AUC)of plasma Presepsin,NGAL,serum PCT and MMP-9 in the diagnosis of secondary multiple organ dysfunction syndrome in emergency sepsis patients was higher than that of PCT,MMP-9,PREPSIN and NGAL alone(P<0.05).Compared with the survival group,the plasma levels of Presepsin,NGAL,serum PCT and MMP-9 in the death group were higher(P<0.05).Conclusion The influencing factors of secondary multiple organ dysfunction syndrome in emergency sepsis patients were the high score of SOFA,levels of plasma PREPSIN,NGAL,serum PCT and MMP-9,and the combined detection of plasma Presepsin,NGAL,serum PCT and MMP-9 had high application value in the clinical diagnosis of multiple organ dysfunction syndrome in emergency sepsis patients.In addition,the above four indexes were related to the prognosis of patients with multiple organ dysfunction syndrome secondary to emergency sepsis.
