The relationship between peripheral blood PLR,NLR,serum Hcy,γ-GT levels and carotid intima-media thickness and plaque stability in elderly patients with essential hypertension

Abstract：Objective To study the relationship between peripheral blood levels of platelet to lymphocyte ratio(PLR),neutrophil to lymphocyte ratio(NLR),serum homocysteine(Hcy),γ-glutamyl transpeptidase(γ-GT)and carotid intima-media thickness and plaque stability in elderly patients with essential hypertension(EH).Methods The clinical data of 152 elderly patients with EH admitted to Pinggu District Hospital of Beijing from March 2023 to July 2024 were retrospectively collected as the case group,and the physical examination data of 152 healthy people who underwent physical examination in Pinggu District Hospital of Beijing during the same period were selected as the control group.The levels of peripheral blood PLR,NLR,serum Hcy and γ-GT were compared between the two groups.According to the different carotid intima-media thickness,152 elderly patients with EH were divided into the normal group(44 cases),the thickening group(51 cases)and the plaque formation group(57 cases).The plaque formation group analyzed the plaque stability group(31 cases)and the plaque instability group(26 cases)according to the difference of plaque stability.The levels of PLR,NLR,serum Hcy and γ-GT in peripheral blood of elderly patients with EH with different carotid intima-media thickness and different plaque stability were compared.The correlation between the four and carotid intima-media thickness in elderly patients with EH and the diagnostic value of carotid plaque stability were analyzed.Results The levels of peripheral blood PLR,NLR,serum Hcy and γ-GT in the case group were higher than those in the control group(t=3.050,22.213,21.392,10.438,P<0.05).The levels of PLR,NLR,Hcy and γ-GT in peripheral blood of normal group,thickening group and plaque group showed an increasing trend(F=53.123,134.167,54.042,53.095,P<0.05).The levels of peripheral blood PLR,NLR,serum Hcy and γ-GT in unstable plaque group were higher than those in stable plaque group(t=3.679,4.157,3.484,4.541,P<0.05).The levels of peripheral blood PLR,NLR,serum Hcy and γ-GT were positively correlated with carotid intima-media thickness in elderly patients with EH(r=0.503,0.477,0.619,0.581,P<0.05).The area under the curve(AUC)of peripheral blood PLR,NLR,serum Hcy and γ-GT in the diagnosis of carotid plaque instability in elderly patients with EH was higher than that of the four alone(P<0.05).Conclusion The levels of peripheral blood PLR,NLR,serum Hcy and γ-GT in elderly EH patients were increased,and the four were significantly correlated with carotid intima-media thickness.The combined diagnosis of the four has a high reference value for the instability of carotid plaque.
