Application of transcranial magnetic stimulation of intermittent Theta rhythm in rehabilitation of dysarticulation after stroke

Abstract:Objective To analyze the application effect of intermittent theta burst stimulation(iTBS)in the rehabilitation of dysarthria after stroke(DAS).Methods A total of 98 patients with DAS admitted to the Department of Neurorehabilitation of our hospital from July 2022 to July 2024 were prospectively selected and randomly divided into the iTBS group(n=49)and the control group(n=49).Both groups received speech therapy,with the iTBS group receiving iTBS and the control group receiving sham iTBS.Both groups were treated for 4 weeks.The therapeutic effects,comprehensive evaluation of dysarthria,vocal conditions,voice quality and communication ability were compared between the two groups before and after 4 weeks of rehabilitation treatment.Results After 4 weeks of rehabilitation treatment,the total effective rate of the iTBS group was higher than that of the control group[85.71%(42/49)vs 65.31%(32/49)],(χ2=5.518,P<0.05).After 4 weeks of rehabilitation treatment,the scores of each dimension of the comprehensive evaluation of dysarthria,voice disturbance index(VHI),and grade,roughness,breathiness,asthenia,strain(GRBAS)scale decreased in both groups,and the iTBS group was lower than the control group(t=4.906,12.670,7.482,6.503,9.523,12.208,7.988,12.756,11.264,12.714,P<0.05).After 4 weeks of rehabilitation treatment,the scores of speech clarity,speech loudness,communication effectiveness index(CETI),and maximum phonation time(MPT)increased in both groups,and the iTBS group was higher than the control group(t=11.649,2.134,21.649,7.398,P<0.05).Conclusion iTBS combined with speech therapy could significantly improve the speech function of DAS patients,and its therapeutic effect was superior to that of false iTBS combined with speech therapy,and it showed more significant improvement in vocal conditions,voice quality and communication ability,and might be an effective auxiliary means for the rehabilitation treatment of dysarthria after stroke.
