The functional improvement effect of edaravone dextran combined with exercise rehabilitation on acute cerebral infarction

Abstract:Objective To observe the improvement of limb and neurological function in the acute phase of cerebral infarction by edaravone dextran combined with modified mandatory exercise.Methods Ninety-five patients in the acute stage of cerebral infarction admitted between May 2021 and December 2024 were selected and randomly divided into two groups using the envelope method.Both groups were given intravenous thrombolytic therapy within the time window,idaravone dextran combined with conventional early rehabilitation therapy in the conventional group,and idaravone dextran combined with modified mandatory exercise therapy in the modified group.The two groups were compared in terms of NOD-like receptor pyrin domain containing 3(NLRP3)signaling pathway,hemodynamics of the middle cerebral artery on the side of the lesion,Carroll's upper ex-tremities function test(UEFT)score,and modified exercise therapy.UEFT score,modified Barthel(BarthelIndex,BI)index,Fugl-meyer Motor Function Upper Extremities(Fugl-meyer Simple Upper Limb Motor Function Score,FMA-UE)score,neurologic function score,Ashworth score,and differences that the total clinical efficiency of the two groups was counted.Results After treatment,NLRP3 mRNA and ASC mRNA were lower in both groups compared with before treatment,and the modified group was lower than the conventional group after treatment(t=23.706,12.392,P<0.05).Mean blood flow velocity(Vm),peak systolic flow rate(Vs),and end-diastolic blood flow velocity(Vd)of the middle cerebral artery on the side of the lesion were higher in both groups after treatment compared with before treatment,and the modified group were higher than in the conventional group after treatment(t=-2.540,-3.372,-3.451,P<0.05).Pulse index(PI)of the middle cerebral artery on the side of the lesion was lower than that before treatment in both groups,and the modified group was lower than that of the conventional group(t=2.280,P<0.05).The FMA-UE scores,BI index,and UEFT scores of both groups were higher after treatment compared with those before treatment,and the modified group were higher than the conventional group after treatment(t=-5.194,-8.376,-5.177,P<0.05).Neurological function scores and Ashworth scores were lower in both groups after treatment than before treatment,and the modified group was lower than the conventional group after treatment(t=4.906,7.363,P<0.05).The total clinical effective rate of the modified group(91.67%)was higher than that of the conventional group(76.60%)(χ2=4.057,P=0.044).Conclusion Idaravone dextran combined with modified mandatory exercise promotes recovery of limb function and neurologic function in the acute phase of cerebral infarction and improves outcome.
