The relationship between stress hyperglycemia complicated by malnutrition and hemorrhagic transformation in acute ischemic stroke patients prior to emergency rt-PA thrombolysis

Abstract:Objective To explore the relationship between stress hyperglycemia and hemorrhagic transformation(HT)in patients with acute ischemic stroke prior to emergency rt-PA thrombolysis,this study was conducted.Methods A retrospective selection was made of 264 patients with acute ischemic stroke who developed stress hyperglycemia before receiving emergency rt-PA thrombolytic treatment in our hospital from February 2021 to February 2024 for this study.Patients were partitioned into a HT group and a non-HT group based on the presence or absence of HT within 7 days after thrombolytic treatment.The clinical data of the patients were collected,and the nutritional status of the patients was evaluated by controlling the nutritional status score.Univariate and multivariate logistic regression models were applied to identify the factors contributing to HT following thrombolysis in acute ischemic stroke patients who presented with stress hyperglycemia prior to emergency rt-PA thrombolytic therapy.Results Among the 264 patients with acute ischemic stroke who had stress hyperglycemia before emergency rt-PA thrombolysis,95 cases developed HT after thrombolytic treatment,with an incidence rate of 35.98%.The results of multivariate logistic regression analysis showed that advanced age,a high National Institutes of Health Stroke Scale score upon admission,concurrent hypertension,a history of atrial fibrillation,and malnutrition were independent risk factors for HT in patients with acute ischemic stroke who had stress hyperglycemia before emergency rt-PA thrombolytic treatment(P<0.05).Conclusion AIS patients with stress hyperglycemia before emergency rt-PA thrombolytic therapy have a higher risk of HT.Advanced age,high National Institutes of Health stroke scale score on admission,hypertension,history of atrial fibrillation,and malnutrition are risk factors for HT.
