Study of the correlation between core symptom clusters with quality of life and psychological factors in patients with acute myocardial infarction

Abstract:Objective To analyze the correlation between core symptom clusters,psychological factors and quality of life in patients with acute myocardial infarction(AMI).Methods A total of 200 patients with AMI treated in Jiangsu Provincial People's Hospital from May 2020 to May 2024 were prospectively selected as the research objects by convenient sampling method.The general information questionnaire,World Health Organization Quality of Life-BREF(WHOQOL-BREF),Edmonton Symptom Assessment Scale(ESAS),Hospital Anxiety and Depression Scale(HADS)and Perceived Social Support Scale(PSSS)were selected to investigate the AMI patients.According to the quality of life score,the patients were divided into low quality of life group and high quality of life group.Pearson correlation analysis was conducted to examine the relationships between core symptom clusters,psychological factors,and quality of life in AMI patients.Logistic regression analysis was performed to identify independent factors affecting quality of life.Results The WHOQOL-BREF score of 200 AMI patients was(48.59±4.54)points,which was lower than the norm(55.06±4.95)points(t=16.758,P<0.005).The quality of lfe score of 73 patients was lower than the norm score of healthy people,that is,there were 73 cases in the low quality of life group and 127 cases in the high qualty of life group.Pearson correlation analysis showed that the core symptom clusters and psychological factors of AMI patients were negatively correlated with the quality of life(r=-0.601,-0.632,P<0.005),and the core symptom clusters were positively correlated with psychological factors(r=0.648,P<0.05).Logistic regression analysis showed that a high ESAS score[OR=1.072,95%CI:1.021-1.125],a high HADS score[OR=1.209,95%CI:1.108-1.318],cardiac function grade III-IV at admission[OR=3.177,95%CI:1.261-8.007],family per capita monthly income<5000 yuan[OR=3.288,95%CI:1.412-7.655],and a low PSSS score[OR=0.845,95%CI:0.789-0.905]were independent risk factors for poor quality of life(P<0.05).Conclusion Cardiac function classification at admission,family per capita monthly income,PSSS score,ESAS score and HADS score were the influencing factors of low quality of life in AMI patients.The core symptom group and psychological factors of AMI patients were negatively correlated with quality of life.
