Effects of staged cardiac rehabilitation training combined with extracorporeal membrane oxygenation on cardiopulmonary function,hemodynamics and quality of life in patients with acute myocardial infarction complicated with cardiogenic shock

Abstract:Objective To investigate the effects of staged cardiac rehabilitation training combined with extracorporeal membrane oxygenation(ECMO)on cardiopulmonary function,hemodynamics and quality of life in patients with acute myocardial infarction complicated with cardiogenic shock(AMI-CS).Methods A total of 96 patients with AMI-CS who were treated in our hospital from March 2022 to March 2024 were prospectively selected and divided into control group(48 cases)and combined group(48 cases)according to the random number table method.Both groups were treated with ECMO.The control group was given routine cardiac rehabilitation intervention on this basis,and the combined group was given phased cardiac rehabilitation training on this basis.The cardiopulmonary function[oxygen pulse,maximum oxygen uptake(VO2max),anaerobic threshold,left ventricular ejection fraction(LVEF),left ventricular end-systolic diameter(LVESD),left ventricular end-diastolic diameter(LVEDD)],hemodynamics[mean arterial pressure(MAP),central venous pressure(CVP),arterial systolic blood pressure(SBP),pulmonary artery wedge pressure(PAWP)],6-minute walking test(6 MWT),36-item short form health survey(SF-36)score and incidence of adverse cardiovascular events(MACE)were compared between the two groups.Results After the treatment,the oxygen pulse,VO2max,anaerobic threshold,left ventricular ejection fraction(LVEF),mean arterial pressure(MAP),systolic blood pressure(SBP),6-minute walk test(6MWT),and SF-36 score of the observation group were all higher than those of the control group(t=2.587,5.564,5.273,4.175,4.576,18.582,5.426,P<0.05).While the left ventricular end-systolic diameter(LVESD),left ventricular end-diastolic diameter(LVEDD),central venous pressure(CVP),and pulmonary artery wedge pressure(PAWP)were all lower than those of the control group(t=12.348,6.982,4.092,11.889,P<0.05).During the treatment,the incidence of MACE in the combined group was lower than that in the control group(χ2=4.376,P<0.05).Conclusion The use of staged cardiac rehabilitation training combined with ECMO in patients with AMI-CS can significantly improve their cardiopulmonary function and hemodynamics,improve exercise tolerance and quality of life,and reduce the incidence of MACE.
