Risk factors for pulmonary vein stump thrombus in patients with left-sided lung cancer after pulmonary lobectomy

Abstract:Objective To investigate the risk factors for pulmonary vein stump thrombus(PVST)in patients with left-sided lung cancer after pulmonary lobectomy.Methods A total of 192 patients with left-sided lung cancer who underwent pulmonary lobectomy in the hospital from March 2022 to March 2024 were selected.Their relevant data were collected,and the incidence of postoperative PVST was statistically analyzed.The patients were divided into PVST group and non-PVST group according to whether PVST occurred.Relevant data of the two groups were compared,and binary logistic regression analysis was performed to screen risk factors for PVST.Results In this study,55 patients developed PVST after surgery,with an incidence rate of 28.65%.General information,tumor node metastasis(TNM)classification,pathological type,hypertension,hyperlipidemia,coronary heart disease,history of atrial fibrillation,preoperative carcinoembryonic antigen(CEA),preoperative D-dimer,preoperative interleukin-6,surgery time,intraoperative blood loss,neoadjuvant therapy,and postoperative atrial fibrillation displayed no statistically significant differences between the two groups(P>0.05).However,the proportions of patients with diabetes and patients undergoing left upper lobectomy in PVST group(85.45%and 74.55%)were significantly higher than those in non-PVST group(25.55%and 54.01%)(χ2=7.587,6.887,P<0.05).The proportion of patients receiving antithrombotic drug treatment before surgery(32.73%)was lower than that in non-PVST group(75.18%)(χ2=5.509,P=0.001),and pulmonary vein stump[(2.47±0.29)mm]was longer than that in non-PVST group[(2.27±0.28)mm](t=4.429,P=0.000).Binary logistic regression analysis showed that diabetes and the length of pulmonary vein stump were risk factors for PVST in patients with left lung cancer after pulmonary lobectomy(P<0.05).Conclusion The risk of PVST in patients with left-sided lung cancer after pulmonary lobectomy deserves attention in clinical practice.The occurrence of PVST is closely related to diabetes,preoperative antithrombotic drug treatment,site of lesion resection,and the length of pulmonary vein stump.
