Application of LIPS and serum FGF-21 in the evaluation of disease condition and prognosis in patients with acute respiratory distress syndrome

Abstract：Objective To study the application effect of lung injury prediction score(LIPS)and serum fibroblast growth factor 21(FGF-21)levels on the disease condition and prognosis in patients with acute respiratory distress syndrome(ARDS).Methods Totally 100 patients with ARDS from July 2023 to July 2024 were retrospectively selected,and the clinical data of patients were collected.The above patients were grouped according to the disease severity and prognosis.Univariate analysis was used to compare the LIPS and serum FGF-21 in patients with different disease condition and prognosis groups.The risk factors of disease condition and prognosis were explored by multivariate logistic regression analysis,and the areas under the curves(AUCs)of LIPS and FGF-21 on disease condition and prognosis were calculated through the receiver operating characteristic curve(ROC).Results The patients were classified into mild group(n=41),moderate group(n=35)and severe group(n=24).The hemoglobin(Hb)in the mild group was the highest,and the levels of FGF-21,serum creatinine(Scr)and LIPS in the mild group were the lowest(F=8.679,107.936,7.195,24.242,P<0.05).By means of different prognosis status,patients were divided into the survival group(n=68)and the death group(n=32).The mechanical ventilation rate,white blood cell count(WBC),FGF-21,Scr and LIPS in the survival group were all lower than those in the death group(χ2=5.933,t=2.469,5.844,3.000,5.084,P<0.05).There was a significant difference in the causes of ARDS between the survival group and death group(Fisher's exact probability<0.001).Multivariate Logistic regression analysis revealed that the increase of FGF-21 level was independently associated with disease deterioration(OR=1.080,95%CI:1.046～1.115,P<0.001),and elevations of LIPS and FGF-21 are independently correlated with death of patients(OR=3.081,95%CI:1.127～8.419;OR=1.056,95%CI:1.020～1.094;P<0.05).ROC curve found that the AUC of FGF-21 combined with LIPS in predicting the prognosis of ARDS patients was 0.812,suggesting that the combination of the two had good predictive efficiency on the prognosis of ARDS patients.Conclusion Serum FGF-21 is closely related to the disease severity of ARDS patients,and the combination of FGF-21 and LIPS has good evaluation value on evaluating the prognosis of ARDS patients.
