Relationship between miR-410,circulating exosomal PD-L1 and surgical prognosis of patients with osteosarcoma and construction of prediction model

Abstract:Objective To analyse the relationship between microRNA-410(miR-410),circulating exosomal programmed cell death ligand 1(PD-L1)and surgical prognosis of patients with osteosarcoma,and to further construct a prognostic prediction model for patients.Methods Retrospective analysis of 50 cases of primary osteosarcoma patients admitted to our hospital from January 2019 to January 2024 as a case group,and the data of age-sex-matched healthy people admitted for physical examination during the same period as a healthy group,to collect the 2 groups of circulating exosomal programmed cell death ligand 1(PD-L1),microRNA-410(microRNA-410,miR-410)expression levels;among them,the patients in the observation group were treated with neoadjuvant chemotherapy after surgical resection of primary osteosarcoma,and the COX regression model analysis model was used to study the influencing factors of the patients'surgical prognosis and to further follow up for 1 year to observe their OS and PFS survival,and to fit the Kaplan-Meier curves to the circulating exosomes.PD-L1 percentage ratio,1-year overall survival of CC patients in miR-410 low and high expression groups,as analysed by www.targetscan.org/Bioinformatics analysis of miR-410 targeting relationship with genes encoding PD-L1.Results Compared with the healthy group,the preoperative PD-L1 and miR-410 levels of patients in the osteosarcoma group were significantly higher than those in the healthy group(t=25.882,17.889,P<0.001).Cox model regression analysis showed that distant metastasis(HR=1.256,95%CI:1.125~1.328,P=0.023),surgical staging(HR=1.539,95%CI:1.467~1.672,P=0.019),preoperative PD-L1(HR=3.061,95%CI:1.894~6.072,P=0.013),and preoperative miR-410(HR=2.184,95%CI:1.076~4.372,P=0.007)were all closely associated with the occurrence of CR obtained in the early postoperative period(P<0.05).www.targetscan.org/Bioinformatics analysis showed that there was a targeting relationship between miR-410 and SPDL1,the gene coding for PD-L1;weighted Kaplan-Meier analysis revealed that patients with high miR-410 expression and high percentage of circulating exosomal PD-L1 had high risk of OS and PFS death(Log rank P<0.05).Conclusion Osteosarcoma patients have obvious high circulating exosomal PD-L1 and high preoperative miR-410 expression,and patients with higher preoperative PD-L1 and miR-410 levels may predict poorer short-term outcomes as their independent risk factors.miR-410 targets and regulates SPDL1,the gene encoding circulating exosomal PD-L1,which explains the patients 1-year follow-up OS and PFS mortality The risk is high,and the clinic should take miR-410 and circulating exosomal PD-L1 into consideration as a new tool that the clinic can evaluate the prognosis of osteosarcoma patients for the early identification of high-risk patients and adjust the particular advantages of the treatment regimen.
