Analysis of core symptoms and influencing factors of gastrointestinal symptom clusters in patients with esophageal cancer during concurrent chemoradiotherapy

Abstract:Objective To investigate the core symptoms and influencing factors of gastrointestinal symptom clusters in patients with esophageal cancer during concurrent chemoradiotherapy.Methods The convenience sampling method was used to prospectively select 226 patients with esophageal cancer who received concurrent chemoradiotherapy in our hospital from August 2023 to November 2024.The Chinese version of Anderson's Digestive Tract Cancer Symptom Assessment Scale was used to evaluate the symptom clusters of patients with esophageal cancer and exploratory factor analysis was used to extract the gastrointestinal symptom clusters.The general information questionnaire,self-rating anxiety scale(SAS)and self-rating depression scale(SDS)were used to investigate.Univariate and multivariate logistic regression were used to analyze the influencing factors of core symptoms of gastrointestinal symptom clusters in patients with esophageal cancer during concurrent chemoradiotherapy.Results The gastrointestinal symptoms in 226 patients with esophageal cancer were nausea 53.54%(121/226),vomiting 50.44%(114/226),loss of appetite 34.96%(79/226),and constipation 24.78%(56/226).The severity scores of each symptom in the gastrointestinal symptom group were(5.67±1.24)points,(5.52±1.21)points,(4.24±1.05)points,and(2.97±0.86)points,respectively.The scores of nausea and vomiting were higher than those of loss of appetite and constipation,and the difference was statistically significant(t=8.458、14.727、7.621、14.108,P<0.05).There was no significant difference in nausea and vomiting scores(t=0.938,P>0.05).Multivariate logistic regression analysis showed that younger age,no drinking history,chemotherapy cycle≥3 times,lymph node metastasis,high SAS score and high SDS score were independent risk factors for nausea and vomiting during concurrent chemoradiotherapy in patients with esophageal cancer(P<0.05).Conclusion Nausea and vomiting are the core symptoms of gastrointestinal symptom clusters during concurrent chemoradiotherapy in patients with esophageal cancer.Young age,no drinking history,chemotherapy cycle≥3 times,lymph node metastasis,high SAS score,and high SDS score are independent risk factors for nausea and vomiting during concurrent chemoradiotherapy in patients with esophageal cancer.
