Study on the Improvement of Postoperative Urethral Pain and Irritation Symptoms in Elderly Male Patients Undergoing Ureteroscopic Lithotripsy with Lidocaine Gel Catheterization
Abstract:Objective To investigate the effect of lidocaine gel catheterization on postoperative urethral pain and irritation symptoms in elderly male patients undergoing ureteroscopic lithotripsy. Methods A total of 86 elderly male patients who underwent ureteroscopic lithotripsy at Pingu Hospital, Beijing Friendship Hospital, Capital Medical University from May 2021 to May 2024 were retrospectively selected. According to the type of lubricant used, they were divided into a control group (39 cases) and an observation group (47 cases). The control group used conventional sterile paraffin oil as a lubricant, while the observation group used lidocaine gel. The urethral irritation response, urethral pain level, urethral mucosal injury degree, and urethral irritation symptoms were compared between the two groups. Results There were statistically significant differences in age (72.12±2.45 vs. 73.98±2.67 years), body weight (71.32±2.64 vs. 72.68±2.75 kg), and operation time (68.12±2.68 vs. 66.38±2.57 min) between the control group and the observation group (x2=3.338, 2.325, 3.066, P<0.05). There was no statistically significant difference in the American Society of Anesthesiologists (ASA) physical status classification (P>0.05). At 6 hours postoperatively, the number of patients with 0-level urethral irritation reaction in the observation group was 42, significantly higher than 4 in the control group, and the 1-4 levels were all lower than those in the control group (x2=55.729, P=0.000). At 12 hours postoperatively, the number of patients with 0-level urethral irritation reaction in the observation group was 39, which was higher than 5 in the control group, and the 1-4 levels were significantly lower (x2=45.357, P=0.000). In the observation group, 13 patients had 0-level catheterization pain and 22 patients had 1-level pain, which were significantly higher than 0 and 9 in the control group, respectively, and grades II-IV were lower in the observation group (x2=29.534, P=0.000). The number of patients with 0-level and 1-level urethral irritation symptoms in the observation group was 25 and 18, respectively, both higher than in the control group, and the grades II-IV were significantly lower (x2=41.622, P=0.000). Conclusion Lidocaine gel can significantly reduce postoperative urethral irritation response and urethral pain, reduce urethral mucosal injury, and improve urethral irritation symptoms.
