Investigation of the correlation between coagulation disorder score and in-hospital mortality in patients with acute respiratory failure

Abstract:Objective To investigate the relationship between coagulation disorder score and prognosis in patients with acute respiratory failure(ARF),and evaluate its clinical value in predicting in-hospital mortality.Methods A retrospective analysis was conducted on 412 ARF patients from Yilong County People's Hospital.The coagulation disorder score(0~6 points)was calculated based on platelet count,international normalized ratio(INR),and activated partial thromboplastin time(APTT).Logistic regression was used to analyze the relationship between the coagulation disorder score and in-hospital mortality.Receiver operating characteristic(ROC)curves were used to compare the predictive performance of this score with Sequential Organ Failure Assessment(SOFA)and Simplified Acute Physiology Score Ⅱ(SAPS Ⅱ).Results In-hospital mortality increased significantly with higher coagulation disorder scores(71.4%vs 19.6%)(P<0.05).Multivariate analysis showed that higher coagulation disorder scores were significantly associated with increased risk of in-hospital mortality(OR=6.88,95%CI:3.92~12.06,P<0.001).Each one-point increase in the score was associated with a 21%increase in mortality risk(OR=1.21,95%CI:1.15~1.27,P<0.001).ROC curve analysis showed that the predictive performance of this score(AUC=0.759)was superior to both SOFA and SAPS Ⅱ scores(P<0.05).Conclusion The coagulation disorder score can serve as an effective tool for predicting in-hospital mortality risk in ARF patients,with superior predictive performance compared to traditional scoring systems and advantages of simple clinical application.
