The effect of Lee Silverman voice treatment combined with electromyographic biofeedback on swallowing function,voice quality and surface electromyography in patients with dysphagia after cerebral infarction

Abstract:Objective To investigate the effects of Lee Silverman voice treatment(LSVT)combined with electromyographic biofeedback(EMGBF)on swallowing function,voice quality and surface electromyography in patients with dysphagia after cerebral infarction.Methods A total of 158 patients with dysphagia after cerebral infarction who were treated in our hospital from June 2022 to June 2024 were prospectively selected and divided into control group and study group by random number table method,with 79 cases in each group.The patients in the two groups were given routine nursing and routine rehabilitation training.On this basis,the control group was given EMGBF treatment,and the study group was given LSVT treatment on the basis of the control group.All patients were trained continuously for 8 weeks.The efficacy,standard swallowing function assessment scale(SSA)score,water swallowing test(WST)grading,dysphagia specific quality of life scale(SWAL-QOL)score,dysphagia severity assessment scale(DOSS)score,maximum amplitude of suprahyoid muscle group and infrahyoid muscle group,swallowing duration and voice disorder index(VHI)were compared between the two groups.Results After treatment,the total effective rate of the observation group was 91.14%,which was higher than that of the control group(77.22%)(x2=5.750,P<0.05).After treatment,the SSA score[(19.28±2.58)points vs.(24.63±2.89)points],WST grade[(1.85±0.52)grade vs.(2.37±0.66)grade],suprahyoid muscle group swallowing duration[(1.65±0.41)s vs.(2.15±0.59)s],infrahyoid muscle group swallowing duration[(1.68±0.42)s vs.(2.18±0.63)s],VHI total score[(31.45±2.57)points vs.(38.65±2.83)points]and functional dimension[(8.26±0.87)points vs.(10.49±1.24)points],physiological dimension[(15.27±1.28)points vs.(18.72±1.75)points],emotional dimension score[(8.07±0.95)points vs.(10.56±1.27)points]were lower than those in the control group(t=12.274,5.501,6.186,5.869,16.740,13.085,14.143,13.954,P<0.05).The SWAL-QOL score[(118.25±12.72)points vs.(105.36±10.24)points],DOSS score[(5.56±1.12)points vs.(4.43±0.96)points],the maximum amplitude of suprahyoid muscle group[(738.31±53.85)μg vs.(612.91±47.73)μg]and the maximum amplitude of infrahyoid muscle group[(723.94±61.37)μg vs.(619.26±48.75)μg]were higher than those in the control group(t=7.016,6.809,15.489,11.871,P<0.05).Conclusion LSVT combined with EMGBF has a significant effect on patients with dysphagia after cerebral infarction,which can effectively improve their swallowing function,voice quality,maximum amplitude of upper and lower hyoid muscles,swallowing duration and quality of life.
