Incidence and clinical risk assessment of frailty in senile stroke sequelae stage patients

Abstract:Objective Epidemiological investigation of frailty and associated risk determinants in geriatric stroke survivors during chronic post-stroke sequela phase.Methods Using a retrospective questionnaire survey research method,the medical records of 500 elderly patients in the sequelae stage of stroke admitted to our hospital from July 2021 to July 2024 were retrieved,and a questionnaire survey was conducted via telephone.A total of 488 valid questionnaires were collected.Among them,224 patients met the frailty criteria(frailty group),and 264 patients did not meet the frailty diagnostic criteria(non-frailty group).Frailty scores were analyzed across patients in the frailty group with varying demographic characteristics.The demographic data and disease characteristic data of the two groups of patients were compared.Multivariate logistic regression was conducted to identify risk factors associated with frailty.Results The frailty scores of patients aged 75 years and above and those who are dependent on others and unable to take care of themselves were(10.27±2.27)points and(10.55±2.20)points respectively,which were higher than those of patients under 75 years old(8.99±1.98)points and patients who can take care of themselves in daily life(8.67±1.90)points.The differences were statistically significant(t=4.066,-6.447,P<0.05).The results of the Logistic regression model analysis showed that being aged 75 years and above(OR=1.899),being dependent on others for daily life and unable to take care of oneself(OR=1.219),having hemorrhagic stroke(OR=1.533),having a swallowing function less than grade 4(OR=1.606),having hypoproteinemia(OR=3.397),having a National Institutes of Health Stroke Scale(NIHSS)score of 14 points and above at admission(OR=1.956),and having two or more concurrent diseases(OR=1.586)were risk factors for frailty in elderly stroke patients(P<0.05).Conclusion The incidence of frailty among the elderly patients in the sequelae stage of stroke was found relatively higher.This situation is closely associated with factors such as the patients' age,inability to take care of themselves,hemorrhagic stroke,the degree of dysphagia,and hypoproteinemia.
