Clinical observation of continuous lumbar sympathetic block for radicular lower limb pain

Abstract:Objective To investigate the clinical effect of continuous lumbar sympathetic block in the treatment of lower limb radiation pain.Methods The prospective study enrolled 154 patients with lumbar disc herniation and radicular lower limb pain who were admitted to the 2nd Affiliated Hospital of Guizhou University of TCM From January 2021 to January 2024.They were divided into two groups using a random number table,with 77 cases in each group.2 cases in the control group were lost to follow up,and 75 patients were included finally.The control group(n=75)received conventional treatments such as massage and physical therapy.On this basis,the experimental group(n=77)received continuous lumbar sympathetic block.The efficacy,pain,skin temperature,and adverse reactions were compared between the groups.Results Compared with the control group,the effective rate in the experimental group was higher(87.01%vs 72.00%,x2=5.275,P=0.022).After 1 day,3 days,5 days and 10 days of treatment,Visual Analogue Scale(VAS)scores of the experimental group in resting state were significantly lower than those of the control group(t=9.170,13.911,15.370,14.524,P<0.001).After 1 day,3 days,5 days and 10 days of treatment,VAS scores of the experimental group in motion state were significantly lower than those of the control group(t=7.975,17.352,14.455,14.668,P<0.001).After treatment,skin temperature of both groups increased.Skin temperatures of the experimental group after 1 day,3 days and 5 days of treatment were higher than those of the control group(t=24.505,17.968,4.375,P<0.001).Skin temperature of the two groups showed no significant difference after 10 days of treatment(P>0.05).Adverse reactions that occurred during treatment included ataxia,nausea and vomiting,dizziness,and paresthesia.The incidence rates of adverse reactions in the control group and the experimental group were 2.66%and 7.80%,showing no significant difference(Fisher's exact probability=0.276).Conclusion Continuous lumbar sympathetic block can significantly alleviate pain and increase skin temperature of patients with lumbar disc herniation and radicular lower limb pain,demonstrating favorable safety profile with comparable adverse events to conventional therapy.
