Prevalence and correlation of pain crisis and cancer-related fatigue in elderly patients with malignant tumors

Abstract:Objective To investigate the current situation of pain crisis in elderly patients with malignant tumors and analyze its correlation with cancer-related fatigue.Methods A total of 130 elderly patients with malignant tumors who were treated in our hospital from May 2022 to May 2024 were prospectively selected using the convenience sampling method,and were divided into pain crisis group and no pain crisis group according to the presence or absence of pain crisis.The patients' general data were collected,and the digital rating scale(NRS),cancer fatigue scale(CFS),perceived social support scale(PSSS),pain cognition scale for cancer patients(CPI),self-rating depression scale(SDS)and self-rating anxiety scale(SAS)were used to analyze pain and fatigue.Pearson was used to analyze the correlation between pain crisis and cancer-related fatigue and Logistic regression was used to analyze the influencing factors of pain crisis in elderly patients with malignant tumors.Results There were 37 cases of pain crisis in 130 elderly patients with malignant tumors,with an incidence of 28.46%.Pearson correlation analysis showed that the NRS score of elderly patients with malignant tumors was positively correlated with the total CFS score and each dimension score(r=0.648,P<0.05).Logistic regression analysis showed that high CFS score(β=0.625,OR=1.868,95%CI=1.342~2.599),low PSSS score,(β=-0.207,OR=0.813,95%CI=0.702~0.942),high CPI score(β=0.308,OR=1.361,95%CI=1.155~1.604),moderate to severe depression(β=2.218,OR=9.193,95%CI=1.642~51.468),moderate to severe anxiety(β=2.259,OR=9.572,95%CI=1.679~54.555),and radiotherapy and chemotherapy(β=3.803,OR=4.845,95%CI=2.521~9.653)were independent risk factors for pain crisis in elderly patients with malignant tumors(P<0.005).Conclusion The incidence of pain crisis in elderly patients with malignant tumors is relatively high.CFS score,PSSS score,CPI score,anxiety and depression level,and radiotherapy and chemotherapy are the relevant factors of pain crisis in elderly patients with malignant tumors.The level of cancer-related fatigue is positively correlated with the prevalence of pain crisis.
