Diagnostic efficacy of intra-ductal biopsy via PTCD in patients with malignant biliary obstruction

Abstract:Objective To investigate the value of percutaneous transhepatic cholangial drainage(PTCD)through intrabiliary biopsy in the diagnosis of malignant bile duct obstruction.Methods Thirty patients with malignant biliary obstruction admitted to the Emergency General Hospital and Chaoyang Hospital Affiliated to Capital Medical University from February 2021 to May 2024 were included in the retropective study.The diagnostic sensitivity of PTCD-guided biopsy was evaluated in different parts and the incidence of complications.Results The biopsy success rate of the 30 patients included was 100%,and PTCD biopsy pathology showed that the cases of bile duct adenocarcinoma and rhabdomyosarcoma were 23(76.67%)and 1(3.33%),respectively,with a positive rate of 80%.Among 30 patients,23(76.7%)had hilar obstruction and 7(23.3%)had distal common bile duct obstruction.The positive rate of PTCD biopsy for malignant obstruction of hilar bile duct was 87.50%(21/24).Among the 24 positive patients,malignant obstruction of hilar bile duct and malignant obstruction of lower common bile duct were 21 and 3 respectively.The cases of biliary tract infection,high fever,bile extravasation,biliary cardiac reflex,bile duct hemorrhage,and pleural cavity injury were 1(3.33%),1(3.33%),2(6.67%),1(3.33%),1(3.33%),2(6.67%),respectively.Conclusions PTCD-guided biopsy demonstrates high diagnostic accuracy for malignant biliary obstruction(80%positivity),particularly in hilar lesions(87.5%sensitivity).
