Risk factors for osteoporosis in elderly patients with type 2 diabetes mellitus and their associations with urinary albumin excretion rate,creatinine clearance,and 25-Hydroxyvitamin D3

Abstract:Objective To investigate the factors affecting osteoporosis in elderly patients with type 2 diabetes mellitus,and to explore their associations with urinary albumin excretion rate(UAER),creatinine clearance(CCr),and serum 25-hydroxy vitamin D3[25(OH)D3]levels.Methods Ninety-eight elderly patients with type 2 diabetes admitted to our hospital from January 2022 to January 2024 were selected for retrospective study.All patients were divided into a concurrent group(n=40)and a non-concurrent group(n=58)according to whether they had osteoporosis.Multivariate Logistic regression was performed to identify independent risk factors of osteoporosis in elderly patients with type 2 diabetes mellitus.The expression levels of UAER,CCr and 25(OH)D3 were compared between the two groups.To analyze the correlation between UAER,CCr,25(OH)D3 and the factors affecting osteoporosis in elderly patients with type 2 diabetes.Results UAER level in concurrent group was higher than that in non-concurrent group(t=9.896,P=0.000),while CCr,25(OH)D3 level was lower than that in non-concurrent group(t=2.151,16.921;P=0.034,0.000).Univariate Logistic analysis showed that hypersensitive c-reactive protein(hs-CRP)(OR=1.274,P=0.001),osteocalcin(osteocalcin,BGP)(OR=2.924,P=0.001),Hemoglobin A1c,HbAlc(OR=4.927,P=0.001)significantly increased the risk of osteoporosis in elderly patients with type 2 diabetes.Multivariate Logistic analysis showed that serum hs-CRP(OR=1.160,P=0.001)was an independent risk factor for osteoporosis in elderly patients with type 2 diabetes.Correlation analysis showed that UAER level was positively correlated with serum hs-CRP level(r=0.677,P=0.000),and negatively correlated with serum CCr,25(OH)D3 and hs-CRP level(r=-0.217,-0.855;P=0.032,0.000).Conclusion Elevated serum hs-CRP levels are associated with osteoporosis progression in elderly T2DM patients in elderly patients with type 2 diabetes complicated with osteoporosis.The level of HS-CRP is positively correlated with UAER and negatively correlated with serum CCr and 25(OH)D3.
