Prognostic value of advanced glycation end products for exercise ability improvement after cardiac rehabilitation in cardiovascular disease

Abstract:Objective To evaluate the association between skin autofluorescence(SAF),a biomarker of tissue advanced glycation end-product(AGE)accumulation,and post-cardiac rehabilitation(CR)exercise capacity improvement in cardiovascular disease(CVD)patients.Methods We retrospectively analyzed 99 CVD patients undergoing CR from February 2020 to April 2023.The 6-minute walk distance(6MWD),peak oxygen uptake(VO2)and anaerobic threshold(AT)were recorded at baseline and after 3 weeks of CR.Using the AGE Reader in patients with seat from baseline SAF level.Results According to the SAF level tertiles,the patients were divided into low level group(SAF level ≤ 2.56 a.u),medium level group(2.56 a.u<SAF level≤3.37 a.u)and high level group(SAF level>3.37 a.u).There were 33 patients in each group.There were no significant differences in baseline 6MWD,AT and peak VO2 levels among the high,middle and low level groups(P>0.05).After 3 weeks of CR,6MWD(t=2.156,P<0.05),AT(t=2.315,P<0.05)and peak VO2(t=2574,P<0.05)levels in the low level group were higher than those at baseline,but there were no significant changes in the middle level group and the high level group.In addition,the 6MWD(F=3.754,P<0.05),AT(F=6.153,P<0.05)and peak VO2(F=9.250,P<0.05)levels of CVD patients in the low level group after 3 weeks of CR were also higher than those in the medium-level and high-level groups.By the Spearman rank correlation analysis,SAF level and 6 MWDS increase%(rs=0.436,P<0.001),increased the AT%(r,=0.212,P=0.035),increased VO2 peak%(r,=0.297,P=0.003)were negative correlation,After age,sex and correction is still a negative correlation.Conclusion Baseline SAF levels of AGE accumulation in responding tissues were inversely associated with increases in 6MWD,AT,and peak VO2 after CR,indicating that lower baseline SAF levels predict greater CR-induced improvements in exercise capacity.
