Prevalence and risk factors of primary gout and hyperuricemia

Abstract:Objective To investigate the prevalence and risk factors of primary gout and hyperuricemia.Methods A retrospective analysis was conducted of 2221 convalescent patients who admitted in our hospital from August 2023 to August 2024.The convalescent patients with primary gout and hyperuricemia were selected as the observation group,while those without primary gout and hyperuricemia were selected as the control group.Statistically analyze the incidence of primary gout and hyperuricemia in the selected subjects,and use univariate and multivariate logistic regression to analyze the risk factors for the onset of primary gout and hyperuricemia.Results Among the 2221 convalescent patients,there were 35 cases of primary gout,with a prevalence rate of 1.58%.There were 525 cases of hyperuricemia,with a prevalence rate of 23.64%.The total prevalence rate of primary gout and hyperuricemia was 25.21%(560/2221).Sex,drinking history,diabetes,proportion of irregular exercise,and serum uric acid,interleukin-1 β,C-reactive protein,gamma glutamyltransferase levels in the observation group were higher than those in the control group(x2=144.836、17.334、52.118、224.787;t=280.965、87.764、83.102、46.244,P<0.05),the serum estimated glomerular filtration rate level was lower than that of the control group(t=13.024,P<0.001).Multivariate logistic regression analysis showed that gender(OR=1.081,95%CI=1.038~1.127),history of alcohol consumption(OR=1.078,95%CI=1.045~1.112),diabetes mellitus(OR=1.093,95%CI=1.041~1.148),irregular exercise(OR=1.099,95%CI=1.042~1.158),high level of serum SUA(OR=1.070,95%CI=1.035~1.107),high level of serum IL-1β(OR=1.074,95%CI=1.026~1.123),high level of serum CRP(OR=1.103,95%CI=1.036~1.174),high level of serum GGT(OR=1.092,95%CI=1.038~1.149),and low level of serum eGFR(OR=1.071,95%CI=1.034~1.110)were the independent risk factors for the development of primary gout and hyperuricemia(P<0.05).Conclusion The total prevalence rate of primary gout and hyperuricemia in a convalescent group is 25.21%(560/2 221),and the prevalence rate of men is higher than that of women.The incidence is related to drinking history,diabetes,irregular exercise,high levels of serum SUA,IL-1 β,CRP,GGT and low levels of serum eGFR.The results can provide a reference for specific convalescent groups to formulate preventive measures for primary gout and hyperuricemia.
