Risk factors for frailty in patients undergoing maintenance hemodialysis

Abstract:Objective To explore the risk factors for frailty in patients undergoing maintenance hemodialysis(MHD).Methods 197 MHD patients admitted to our hospital from January 2023 to December 2023 were enrolled.According to their frailty status during MHD,patients were assigned to the frailty group and the non-frailty group.Univariate and multivariate logistic regression models were used to summarize the risk factors for frailty in MHD patients.Results The average FRAIL score of the 197 patients was(2.36±0.48).There were 27cases of no frailty,103 cases pre-frailty,and 67 cases of frailty respectively,accounting for 13.71%,52.28%,and 34.01%,respectively.The multivariate logistic regression analysis showed that age ≥ 60 years(OR=1.195,95%CI=1.023~1.395),BMI<18.5 kg/m2(OR=1.301,95%CI=1.032~1.639),dialysis vintage ≥ 36 months(OR=2.852,95%CI=1.831~4.441),ADL score<60(OR=27.330,95%CI=12.800~58.354),ALB<35 g/L(OR=4.100,95%CI=1.034~16.263),CRP ≥ 4 mg/L(OR=2.614,95%CI=1.383~4.943),high comorbidity of CCI(OR=1.674,95%CI=1.206~2.322),and NRS 2002 score ≥ 3(OR=2.477,95%CI=1.068~5.472)were independent risk factors for frailty in MHD patients,and non-living alone(OR=0.065,95%CI=0.022~0.189)was a protective factor(P<0.05).Conclusion The incidence of frailty in MHD patients is high and is affected by various factors such as age,BMI,dialysis vintage,self-care ability,inflammatory status,comorbidities,nutritional status,and living status.According to this,high-risk groups for frailty can be identified.
