Correlation analysis between frailty index and prognosis of elderly heart failure patients within one year after discharge

Abstract:Objective To observe the correlation between frailty index(FI)and readmission and mortality rates within one year after discharge in elderly patients with heart failure.Methods Select elderly patients with chronic heart failure who were hospitalized in the cardiology department from July 2022 to July 2023.Calculate FI and include patients with FI ≥ 0.25 in the frailty group,and patients with FI<0.25 in the non frailty group.Follow up on death due to cardiovascular events and readmission to our hospital or another hospital within 12 months after discharge.Analyze the correlation between FI index and readmission and mortality rates.Results After excluding 273 cases that were lost to follow-up.Compared with the non-frail group,the patients in the frail group were older,had a lower body mass index,were more likely to be female,and had a higher prevalence of comorbid chronic obstructive pulmonary disease,and the differences were statistically significant(t=8.750,4.067,x2=18.72,3.845,P<0.05).There were statistically significant differences in creatinine levels,glomerular filtration rate(eGFR),N-terminal B-type natriuretic peptide precursorNT-proBNP),and NYHA classification between the two groups of patients(t=3.770,9.303,5282,Z=8.830,P<0.05).The readmission rates of 36.71% and 45.31% in the frailty group at 90 days and 1 year were higher than those of the non frailty group at 15.86% and 22.07%,respectively.The mortality rates of 8.59% and 10.94% were higher than those of the non frailty group at 2.76% and 4.82%,respectively.The differences in readmission rates at 90 days,1 year,and 90 days were statistically significant,with x2=15.511,16.621,and 4.457,P<0.05.FI was positively correlated with the readmission rate of patients within 1 year,with a correlation coefficient of 0.482,and had no significant correlation with the mortality rate.Conclusion Weakness affects the prognosis of elderly patients with heart failure,and an increase in frailty index indicates poor prognosis.Comprehensive measures such as strengthening education,nutrition,and exercise intervention should be taken to enhance frailty management and improve patient prognosis.
