Efficacy and impact on the QT interval of electrocardiogram of betaquinoline in the treatment of elderly patients with drug-resistant pulmonary tuberculosis

Abstract:Objective To analyze the efficacy and safety of betaquinoline in treating elderly patients with drug-resistant pulmonary tuberculosis.Methods 128 elderly patients with drug-resistant pulmonary tuberculosis admitted to our hospital from January 2022 to September 2023 were divided into the betaquinoline group and the control group by convenient sampling method,with 64 cases in each group.The control group received traditional drug treatment for drug-resistant tuberculosis,while the observation group used a regimen containing bedaquiline.The negative conversion rate of sputum bacteria and the changes of chest CT manifestations in the two groups were compared.The incidence of adverse reactions in the two groups during treatment was recorded and the treatment outcome of the two groups was evaluated after 24 weeks of treatment.Results 61 patients in the betaquinoline group and 58 patients in the control group were included in the result analysis.The sputum conversion rate of the bedaquiline group after 24 weeks of treatment was 95.08%(58/61),which was higher than the control group' s 82.75%(48/58)(x2=4.639,P=0.031).The lesions in the bedaquiline group showed significant absorption in 17 cases,absorption in 78 cases,and no change in 18 cases,which was higher than the control groups of 6 cases,65 cases,and 35 cases(x2=11.684,P=0.003).The closure of cavities in the bedaquiline group was 18,reduced by 82,and remained unchanged for 13,which was higher than the control groups of 8,71,and 27(x2=9.323,P=0.009).The incidence of QT interval prolongation during treatment in the bedaquiline group was 59.02%,which was higher than that in the control group(32.76%)(x2=8.247,P=0.004).After 24 weeks of treatment,the success rate of the bedaquiline group was 95.08%,which was higher than the control group' s 82.76%(x2=4.639,P=0.031).Conclusion The treatment of elderly drug-resistant pulmonary tuberculosis with a regimen containing bedaquiline has a high sputum conversion rate,which can promote lesion absorption,cavity closure,and improve treatment outcomes.However,it also has a higher incidence of QT interval prolongation,taking comprehensive measures to improve treatment safety.
