The relationship between nutritional status,anxiety and depression and quality of life in patients with dysphagia after stroke was analyzed based on structural equation model

Abstract：Objective Based on the structural equation model,the relationship between nutritional status,anxiety and depression and quality of life in patients with dysphagia after stroke was analyzed.Methods A total of 162 patients with dysphagia after stroke treated in our hospital from January 2020 to December 2023 were selected.Mini-Nutritional Assessment(MNA),self-rating anxiety scale(SAS),and self-rating depression scale(SDS)were used to evaluate nutritional status.Self-rating depression Scale(SDS)and Stroke-Specific Quality of Life Scale(SSQOL)were used to investigate the patients.The structural equation model was used to explore the relationship between nutritional status,anxiety,depression,and quality of life.Results The SSQOL,MNA,SDS and SAS scores of patients with dysphagia after stroke were(125.45±20.25),(10.10±2.67),(77.74±2.44)and(79.61±2.23),respectively.The higher the grade of dysphagia,the lower the SSQOL and MNA scores.The higher the SDS and SAS scores.Nutritional status was positively correlated with quality of life(r=0.745,P＜0.05),while anxiety and depression were negatively correlated with quality of life(r=-0.417,-0.410,P＜0.05).The structural equation model fitted well(x2/df=1.357,GFI=0.882,AGFI=0.849,NFI=0.833,CFI=0.949,RMSEA=0.047).Nutritional status,anxiety and depression had a direct effect on quality of life(path coefficient 0.977,0.156,-0.251,respectively,P＜0.05),and nutritional status had an indirect effect on quality of life(path coefficient 0.064,P＜0.05),in which depression and anxiety played a partial mediating role,and the mediating effect was 6.15％ of the total effect.Conclusion Nutritional status,anxiety and depression are important factors affecting the quality of life of patients with swallowing after stroke.Improving the nutritional status and relieving anxiety and depression can improve the quality of life of patients.
