Analysis analysis of characteristic biochemical indexes in patients with vascular cognitive impairment and senile dementia

Abstract:Objective To explore and analyze the differences of alkaline phosphatase(ALP),neuron specific enolase(NSE),brain derived neurotrophic trophic fac-tors(BDNF),LDL-C content and cerebral blood flow in patients with different severity of vascular cognitive impairment(VCI).Methods 150 patients with VCI admitted to the Department of Neurology of Cangzhou Central Hospital from June 2020 to July 2021 and 50 patients with non-vascular cognitive impairment who underwent physical examination in our hospital during the same period were retrospectively analyzed,and they were set as observation group and control group,respectively.According to the severity of the disease[mini-mental state examination(MMSE)score and montreal cognitive assessment scale(MoCA)score],the patients in the observation group were divided into severe(MMSE≤9 points,MoCA≤10 points),moderate(MMSE 10~21 points,MoCA 11~17 points)and mild(MMSE ≥22 points,MoCA ≥ 18 points).To compare the differences in brain characteristics,ALP,NSE,LDL-C,fasting plasma glucose(FPG),glycosylatedoglobin(HbAlc),homocysteine(Hcy),brain-derived neurotrophic factor(BDNF),and uric acid(UA)and cerebral blood flow(CBF)levels in patients in the observation group and the control group.To compare the differences in the content of serum ALP,NSE,Hcy,LDL-C,FPG,HbAlc,BDNF,UA indicators and the level of cerebral blood flow in patients with vascular cognitive of different severity.The relationship between ALP,NSE,BDNF,LDL-C,FPG,HbAlc,Hcy,UA,ample volume,hook distance,left/right basal CBF,left parietal CBF,left/right temporal lobe CBF,left/right frontal lobe gyr CBF,left/right occipital lobe,right parietal lobe CBF indicators and different severity of the disease was analyzed by variance analysis and multiple regression analysis.Results The contents of ALP,NSE,Hcy,LDL-C,FPG and HbAlc in experimental group were higher than those in control group.The contents of BDNF and UA were significantly lower than those of control group(t=17.477,36.919,-28.167,24.739,19.961,16.177,21.020,-24.334,P<0.05).The hippocampal volume and hook distance of experimental group were lower than those of control group(t=-14.610,-8.368,-2.997,P<0.05).The CBF index levels in the left/right basal,left parietal,left/right temporal and left/right frontal segments of experimental group were lower than those in the control group(t=-0.492,-0.869,-7.173,-30.321,-9.129,-1.359,-3.295,-6.202,-6.464,-8.747,P<0.05).Through multiple linear regression analysis,ALP,NSE,BDNF,LDL-C,FPG,HbAlc,Hcy,UA,left/right basal CBF,left/right temporal CBF,left/right frontal segment CBF were independently correlated with severe vascular cognitive impairment(B=0.067,0.058,-0.056,0.031,0.038,0.023,0.018,-0.022,-0.017,-0.023,-0.073,-0.081,-0.076,-0.065,-0.057,P<0.001).Conclusion ALP,NSE,BDNF,LDL-C,FPG,HbAlc,Hcy,UA,left/right basal CBF,left/right temporal CBF,left/right frontal segment CBF are closely related to severe vascular cognitive impairment.
