Effect of fixed trajectory upper limb robot training on upper limb function and brain function of stroke patients with hemiplegia

Abstract:Objective To analyze the effects of fixed trajectory upper limb robot training on upper limb function and brain function of stroke patients with hemiplegia.Methods From January 2022 to January 2024,102 patients with stroke hemiplegia admitted to the department of neurological rehabilitation of Qingdao Navy Special Service Recuperation Center were prospectively selected and divided into the experimental group and the control group,with 51 cases in each group,using random number table method.Both groups underwent routine rehabilitation.The experimental group received fixed trajectory upper limb robot training and the control group received upper limb robot training with voluntary movement trajectory.Both groups received rehabilitation treatment for 4 weeks.The two groups of upper limb function,quality of life,brain function,cognitive function and Brunnstrom stage of upper limb before rehabilitation treatment,4 weeks after rehabilitation treatment,and the rehabilitation satisfaction after 4 weeks of rehabilitation treatment were compared.Results Compared with before rehabilitation treatment,after 4 weeks of rehabilitation treatment,the scores of fugl meyer assessment upper extremity(FMA-UE),action research arm test(ARAT),wolf motor function test(WMFT),(modified barthel index(MBI)and montreal cognitive assessment(MoCA)in the two groups increased,the experimental groups were(49.65±3.40)points,(47.03±2.25)points,(53.16±5.35)points,(85.29±4.16)points,(15.25±1.43)points,which was higher than that of the control group(41.73±3.35),(41.50±2.24),(43.13±5.32),(76.26±5.55),(12.73±1.42),(t=7.441,12.439,9.494,9.297,8.859,P<0.05).After 4 weeks of rehabilitation treatment,the cortical latency of motor evoked potential(motor evoked potential,MEP)and the central motor conduction time of the two groups were shortened than before rehabilitation treatment,and the experimental group was(21.84±2.86)ms,(10.94±1.57)ms,which was shorter than the control group(24.16±2.74)ms,(12.75±1.42)ms(t=4.183,6.106,P<0.05).Compared with before rehabilitation treatment,the Brunnstrom stage improved in both groups 4 weeks after rehabilitation treatment,and the experimental group,there were 0,13,19,16,3 and 0 cases in stages Ⅰ、Ⅱ、Ⅲ、Ⅳ、Ⅴ and Ⅵ respectively,which were better than 0,6,14,19,8 and 4 cases in the control group(Z=9.866,P<0.05).After 4 weeks of rehabilitation treatment,the total satisfaction rate of rehabilitation in the experimental group was 94.12%(48/51),which was higher than that in the control group(80.39%(41/51)(x2=4.320,P<0.05).Conclusion Compared with voluntary movement trajectory,fixed trajectory upper limb robot training can improve the upper limb function of stroke patients with hemiplegia,improve brain function and cognitive function,optimize the Brunnstrom stage of upper limb,ultimately improving the quality of life and high satisfaction.
