Effect of Edaravone dextrocamphorol on nerve function,cerebral blood perfusion and inflammatory state in acute cerebral infarction

Abstract:Objective To explore the advantages of edaravone dexcamphorol in the treatment of acute cerebral infarction based on the recovery of nerve function,cerebral blood flow perfusion and inflammatory status.Methods A prospective single-center randomized single-blind controlled trial was conducted,A total of 102 patients with acute cerebral infarction admitted to the first department of Workers' Hospital of Qinhuangdao City from August 2022 to July 2023 were randomly divided into the control group(n=106)and the experimental group(n=106).The control group received conventional treatment for cerebral infarction,and the experimental group was given edaravone dextrocamphenol intravenously on this basis.Both groups were treated for 2 weeks.The efficacy after 2 weeks of treatment,neurological function recovery and prognosis of different time,cerebral blood perfusion,inflammatory status,oxidative stress indexes before treatment and 2 weeks after treatment,and safety during treatment were compared between the two groups.Results After 2 weeks of treatment,the total effective rate of the experimental group was higher than that of other groups(96.23% vs.82.08%,x2=10.973,P=0.001).After 2 weeks of treatment and 3 months after treatment,the scores of National Institutes of Health Stroke Scale(NIHSS)and modified Rankin Scale(mRS)in the experimental group were lower than those in the control group[(8.67±1.23)points vs.(10.82±1.56)points,(2.46±0.40)points vs.(2.91±0.42)points,(5.76±1.03)points vs.(7.65±1.33)points,(2.05±0.33)points vs.(2.46±0.32)points](t=11.143,7.988,11.567,9.183,P<0.05).After 2 weeks of treatment,the levels of middle cerebral artery pulse index(PI),serum lipoprotein-associated phospholipase A2(Lp-PLA2),pentamylin 3(PTX3),interleukin-6(IL-6),reactive oxygen species(ROS)and advanced protein oxidation products(AOPP)in the experimental group were lower than those in the control group[(0.67±0.08)vs.(0.81±0.07),(5.93±1.02)μg/L vs.(8.65±1.65)μg/L,(2.50±0.65)ng/L vs.(4.43±0.63)ng/L,(14.12±2.33)ng/mL vs.(16.78±2.25)ng/mL,(2.60±0.62)mol/L vs.(3.48±0.70)mol/L,(11.24±1.24)μmol/L vs.(13.33±1.39)µmol/L](t=13.559,14.436,21.951,8.455,9.689,11.552,P<0.05).After 2 weeks of treatment,the peak systolic blood flow velocity(Vp),average blood flow velocity(Vm),and serum level of glutathione peroxidase(GSH-Px)was higher than that of the control group[(77.54±3.09)cm/s vs.(68.43±3.22)cm/s,(37.55±2.78)cm/s vs.(33.37±2.68)cm/s,(91.43±9.21)U/L vs.(79.76±7.32)U/L](t=21.017,11.145,10.213,P<0.05).The incidence of adverse reactions was similar between the two groups(5.66% vs.7.55%,x2=0.306,P=0.580).Conclusion On top of the usual treatment,edaravone dexcamphorol is effective in the treatment of acute cerebral infarction.It can improve cerebral blood perfusion,reduce inflammatory state and oxidative stress,and then improve neurological function and prognosis with good safety.
