Abstract:Objective To investigate the effects of betahistine hydrochloride combined with anti-vertigo exercises on the severity of vertigo, hemorheology, and vascular endothelial function in patients with vertigo syndrome caused by intracerebral ischemia. Methods This study is a prospective study in which 160 patients with vertigo syndrome due to intracerebral ischemia were prospectively selected from the Department of Neurology in our hospital from January 2022 to February 2024.They were divided into a combination group(n=80)and a control group(n=80)based on a digital randomization table. The control group was treated with oral betahistine hydrochloride tablets, while the combination group received additional anti-vertigo exercise training. The clinical efficacy and 6-month recurrence rate were compared between the two groups. The severity of vertigo before and after treatment[Vertigo Symptom Scale-Short Form(VSS-SF),Berg Balance Scale(BBS),Dizziness Handicap Inventory(DHI)scores],hemorheology[blood viscosity at low shear rate(BVL),medium shear rate(BVM),high shear rate(BVH),plasma viscosity, and hematocrit],and vascular endothelial function[serum nitric oxide(NO),endothelin-1(ET-1)levels]were also compared. Results After treatment, the combination group showed a superior total effective rate in clinical treatment compared to the control group, and a lower recurrence rate(χ2=4.103,8.640,P<0.05).The combination group had significantly lower VSS-SF,DHI,ET-1,BVL,BVM,and BVH indices than the control group(t=0.504,4.679,8.928,9.045,7.282,7.160,P<0.05),and significantly higher BBS scores and NO levels(t=3.588,6.563,P<0.05).Conclusion The treatment of vertigo syndrome caused by intracerebral ischemia with betahistine hydrochloride combined with anti-vertigo exercises is significantly effective with a lower recurrence rate after treatment. It can improve patients'balance, reduce the severity of vertigo, enhance vascular endothelial function, and decrease blood viscosity, with superior improvement outcomes compared to betahistine hydrochloride treatment alone.
