Abstract:Objective To analyze the influencing factors for the progression of acute myocarditis to fulminant myocarditis. Methods Retrospective data were collected from patients with acute myocarditis admitted between January 2016 and August 2024.A total of 187 patients were included, including 48 cases of fulminant myocarditis(FM group)and 139 cases of non-fulminant myocarditis(non-FM group).Symptoms, vital signs, electrocardiogram parameters, echocardiographic indicators, and laboratory test results were compared between the FM group and the non-FM group.Logistic multivariate regression analysis was performed on factors with significant differences. Results Logistic regression analysis showed that prolonged QRS interval, elevated NT-proBNP levels, decreased systolic blood pressure, and reduced LVEF were independent risk factors for the development of fulminant myocarditis. Conclusion Prolonged QRS interval, elevated NT-proBNP levels, decreased systolic blood pressure, and reduced LVEF are independent risk factors for fulminant myocarditis in patients with acute myocarditis.Dynamic monitoring and early identification and intervention for patients with these risks should be emphasized.
