Chain mediation effect of anxiety and depression between exercise self-efficacy and dyspnea-related exercise fear in patients with transnasal high-flow oxygen therapy

Abstract:Objective To examine the correlation between exercise self-efficacy and dyspnea-related exercise fear in patients receiving high-flow nasal cannula oxygen therapy(HFNC),and to explore the chain mediating effects of anxiety and depression.Method A total of 200 patients who received transnasal high-flow oxygen therapy in our hospital from November 2022 to December 2023 were selected.The patients were surveyed using the general data questionnaire,exercise self-efficacy scale,dyspnea belief questionnaire,and hospital anxiety and depression scale.Results Motor self-efficacy was negatively related to dyspnoea-related dyophobia in HFNC(r=-0.593,P<0.001)and anxiety and depression(r=-0.670,-0.0656,P<0.001),and anxiety and depression were positively related to dyskophobia in HFNC(r=0.587,0.559,P<0.001).The direct path analysis revealed that exercise self-efficacy significantly influenced dyspnea-related exercise fear in HFNC patients,with an effect size of-0.425(95%CI:-0.671 to-0.178).The chain-based mediation model results showed significant mediation effects of anxiety and depression between exercise self-efficacy and dyspnea-related kinophobia,with mediation effect sizes of-0.290(95%CI=-0.446~-0.157)and-0.149(95%CI=-0.250~-0.059),respectively;and the chain mediation effect between anxiety and depression was also significant,with-0.072(95%CI=-0.125~-0.027).Conclusions It can reduce the dyspnea-related exercise fear by improving the exercise self-efficacy of HFNC patients,while the exercise self-efficacy can also relieve the dyspnea-related exercise fear in the patients by reducing anxiety and depression.
