Safety and efficacy analysis of 150W-450 nm blue laser vaporization surgery for the treatment of elderly prostate hyperplasia

Abstract:Objective To explore the clinical efficacy and safety of 150W-450 nm blue laser vaporization surgery for treating elderly benign prostatic hyperplasia(BPH).Methods A retrospective analysis was conducted on the clinical data of 60 BPH patients aged≥80 years old in our hospital from January 2023 to January 2024.They were divided into two groups according to surgical methods.The observation group(n=30)underwent transurethral 450 nm blue laser vaporization of the prostate,while the control group(n=30)underwent transurethral resection of the prostate(TURP).The surgical time,intraoperative and postoperative blood loss,duration of bladder irrigation and catheterization,incidence of complications,improvement in postoperative quality of life,and satisfaction were compared between the two groups.Preoperative and postoperative bladder residual urine volume measurement(PVR),maximum urine flow rate(Qmax),and International Prostate Symptom Score(IPSS)were also measured.Results All 60 surgeries were successfully completed.Compared to the TURP control group,the transurethral 450 nm blue laser vaporization surgery had a shorter surgical time,less intraoperative and postoperative bleeding,the duration of postoperative bladder irrigation is shorter,the postoperative indwelling catheterization time is shorter(t=15.373,9.907,9.515,29.818,P<0.05).The incidence of surgical complications was lower(6.67%vs.33.33%,P=0.01),and at 3 months after surgery,the IPSS score of patients was significantly lower than before surgery,Qmax was higher than before surgery,PVR was significantly reduced,perioperative satisfaction and quality of life were higher than the control group,with statistical significance(P<0.05).Conclusion Transurethral 450 nm blue laser vaporization for treating BPH has the characteristics of less bleeding,shorter surgical time,less side damage,faster recovery,shorter bladder flushing and catheter retention time,improved patient satisfaction and quality of life compared to TURP.It is safe and efficient,and is worthy of clinical promotion and application for elderly BPH patients.
