Correlation analysis of prognosis nutrition index and hemorrhage transformation after intravenous thrombolysis in elderly patients with acute ischemic stroke

Abstract:Objective To explore the correlation of prognosis nutrition index(PNI)and intravenous thrombolysis hemorrhagic conversion(HT)in elderly patients with acute ischemic stroke(AIS).Methods A total of 158 elderly AIS patients treated with alteplase intravenous thrombolysis in Hai'an People's Hospital from February 2021 to 2 February 2023 were divided into HT group and no HT group according to the occurrence of HT.Multivariate Logistic regression was used to analyze the influencing factors of HT in elderly AIS patients,and Spearman correlation was used to analyze the association between PNI and HT.Results The incidence of HT in the 158 elderly patients with AIS was 27.22%(43/158).Logistic Regression analysis showed that NIHSS score at admission,onset to thrombolytic therapy,combined hypertension,atrial fibrillation history and PNI were influencing factors for HT in elderly patients with AIS(P<0.05).Spearman correlation analysis showed that PNI was negatively associated with hemorrhagic transformation after intravenous thrombolysis in elderly AIS patients(r=-0.317,P<0.001).The ROC curve analysis showed that the AUC of PNI in predicting the prognosis of elderly AIS patients was 0.792(95%CI=0.719~0.865).Conclusion NIHSS score at admission,time of onset to thrombolytic therapy,history of combined hypertension and atrial fibrillation are the influencing factors of HT after intravenous thrombolysis in elderly patients with AIS,and PNI is negatively associated with HT,which could provide reference value for patient prognosis in clinical diagnosis.
