Efficacy of dapagliflozin,metformin combined with insulin in the treatment of elderly T2DM and its effect on vaspin and inflammatory factors

Abstract：Objective To investigate the efficacy of dapagliflozin,metformin combined with insulin in the treatment of elderly patients with type 2 diabetes mellitus and its effect on serum lipid-specific serine protease inhibitor(vaspin)and inflammatory factors.Methods A total of 106 elderly patients with type 2 diabetes mellitus admitted to our hospital from May 2022 to May 2023 were retrospectively selected as the research objects.According to the treatment plan,they were divided into metformin group(n=50)and combination group(n=56).Both groups were given routine insulin to maintain basic blood glucose.The metformin group was treated with metformin hydrochloride sustained-release tablets,and the combination group was treated with dapagliflozin tablets on the basis of the metformin group.The blood glucose level,insulin index,serum vaspin and inflammatory factor levels were compared between the two groups.Results The FPG,2 hPG,HbA1 c,vaspin,IL-1,IL-6,hs-CRP and TNF-α indexes of the two groups after treatment were lower than those before treatment.Preprandial C-P and postprandial 1 h C-P were higher than those before treatment.After treatment,FPG,2 hPG,HbA1 c,vaspin,IL-1,IL-6,hs-CRP and TNF-α in the combined medication group were significantly lower than those in the metformin group(t=5.546,73496,2.168,2.148,4.424,25.959,16.158,7.217,P<0.05).Preprandial C-P and 1 h postprandial C-P were higher than those in the metformin group(t=10.150,3.864,P<0.05).Conclusion For elderly patients with type 2 diabetes,dapagliflozin combined with metformin can well control blood glucose levels,reduce serum Vaspin and related inflammatory factor levels,and increase C-peptide levels,which can provide a certain reference value in clinical practice.
