Predictive value of systemic immune-inflammation index for recurrence and metastasis in patients with locally advanced esophageal cancer

Abstract：Objective To analyze the predictive value of the systemic immune-inflammation index(SII)in predicting recurrence and metastasis in patients with locally advanced esophageal cancer.Methods 160 patients with esophageal cancer who treated in our hospital from May 2018 to August 2022 were included.According to the recurrence or metastasis of esophageal cancer after treatment,they were divided into occurrence group(68 cases)and non-occurrence group(92 cases).The general data and clinical data were collected retrospectively.Logistic regression was used to analyze the risk of recurrence and metastasis in patients with locally advanced esophageal cancer.ROC curve was used to evaluate the predictive value of SII for the risk of recurrence and metastasis in patients with locally advanced esophageal cancer.Results There were statistically significant differences in TNM staging,tumor differentiation degree,tumor diameter,and systemic immune inflammation index between the two groups of patients(t=12.711,20.807,6.901,6.455,P<0.05).The TNM staging,tumor differentiation degree,and tumor diameter data of patients with high levels of SII were significantly higher than those of patients with low levels,(t=21.107,14.731,14.204,P<0.05).Multivariate logistic regression analysis showed that TNM stage,depth of tumor invasion,degree of tumor differentiation,tumor diameter and SII were independent influencing factors of recurrence and metastasis in patients with locally advanced esophageal cancer.SII was used as a continuous variable,and ROC curve was used to analyze the predictive effect of SII on recurrence and metastasis in patients with locally advanced esophageal cancer.The results showed that the area under the curve of blood SII was 0.811(0.752～0.872),the sensitivity was 0.632,and the specificity was 0.881.Conclusion The SII is an independent factor influencing recurrence and metastasis in patients with locally advanced esophageal cancer,effectively predicting the risk of recurrence and metastasis in this population.
