Analysis of influencing factors on in-stent restenosis after coronary stent implantation

Abstract:Objective To analyze the influencing factors of in-stent restenosis(ISR)after drug-eluting stent(DES)implantation in coronary heart disease.Methods Patients who underwent percutaneous coronary stent implantation from April 2020 to March 2022 and had complete follow-up data were included consecutively.According to the follow-up results of coronary angiography,873 cases were divided into a non-ISR group,and 104 cases were in the ISR group.The general clinical data,laboratory and other auxiliary examinations,lesion location,lesion type,presence of calcification and other vascular conditions,and stent-related indicators such as the number,length,diameter,and serial connection of stents were compared between the two groups.Multivariate logistic regression analysis was performed.Results Smoking,poor medication adherence,TC,LDL-C,UA,regular exercise,bifurcation lesions,moderate to severe calcification,and stent diameter,which showed statistical differences in univariate analysis,were included in the logistic regression analysis.After removing collinearity,smoking,poor medication adherence,high LDL-C levels,bifurcation lesions,moderate to severe calcification,and small stent diameter were identified as risk factors for ISR,while regular exercise was a protective factor for ISR(P<0.05).Conclusion Bifurcation lesions,coronary artery calcification,thin stents,elevated LDL-C levels,patient smoking,irregular exercise,and poor medication adherence are risk factors for ISR in patients with coronary heart disease who have undergone DES implantation.Targeted strategies should be adopted to prevent ISR.
