A nomogram model construction of secondary cognitive dysfunction in patients with septic shock

Abstract:Objective Decision tree(CHAID)and dichotomous logistic regression were used to predict risk factors for cognitive dysfunction secondary to sepsis and to construct a risk prediction model.Methods The clinical data of 429 sepsis patients from March 2020 to April 2022 were selected for analysis,The CHAID algorithm and binary logistic regression analysis were used to analyze the risk factors for sepsis-related cognitive dysfunction.The receiver operating characteristic(ROC)curve was utilized to compare the predictive performance of the two models.Results By logistic regression analysis,Age(OR=2.025,95%CI=1.720~2.381),weak(OR=-1.196,95%CI= 0.118~0.778),poor sleep quality(OR= 2.027,95%CI= 2.350~24.510),alcohol consumption(OR=9.221,95%CI = 2.115~40.200),duration of delusion(OR= 1.883,95%CI = 2.291~18.868)was an independent risk factor for cognitive dysfunction secondary to septic shock(P<0.05).Decision tree model:false duration 2 d,poor sleep quality,analgesic drug application,frailty are four important explanatory variables in the decision tree sepsis related cognitive dysfunction,the incidence of false duration 2 d 100.00%,poor sleep quality 56.9%,59.3%,16.9%.Conclusion Both models highlighted frailty,poor sleep quality,and delusion duration as risk factors for sepsis-related cognitive dysfunction,and caregivers should conduct early intervention in patients with risk factors to avoid an elevated risk of sepsis-related cognitive dysfunction.
