Risk factors of secondary postoperative respiratory complications in patients undergoing radical gastrectomy for gastric cancer

Abstract:Objective To investigate the risk factors of postoperative respiratory complications(PRCs)in patients undergoing radical gastrectomy for gastric cancer.Methods The clinical data of 159 patients who underwent radical gastrectomy for gastric cancer in our hospital from January 2018 to January 2022 were retrospectively analyzed,and the incidence of PRCs was calculated.The related factors of PRCs in patients with gastric cancer were analyzed,and the risk prediction model of PRCs was constructed.The accuracy of the prediction model was evaluated by receiver operating characteristic curve(ROC).Results 41 patients with gastric cancer developed PRCs after surgery,with an incidence of 25.79%.Multivariate Logistic regression analysis showed that age≥75 years old(X1),smoking history(X2),hypoalbuminemia(X3),history of lung disease(X4)and open surgery(X5)were independent risk factors for PRCs in patients with gastric cancer(P<0.05).The PRCs prediction model constructed by the screened high-risk factors was logit(P)= 0.975X1+1.107X2+0.957X3+2.197X4+1.529X5-2.938.The area under the curve(AUC)of the model for predicting PRCs in patients with gastric cancer was 0.828(95%CI = 0.765~0.891),the predictive sensitivity was 90.2%,and the specificity was 61.0%.Conclusion The risk of PRCs after radical gastrectomy is high.Advanced age(≥75 years),smoking history,hypoalbuminemia,history of lung disease and open surgery are independent risk factors for PRCs in patients with gastric cancer.The prediction model constructed by combining risk factors is helpful to evaluate the risk of PRCs in clinical practice.
